Reading the OP again,
if the fish has only recently developed swimming and balance problems, then its probably because it has reached sexual maturity.

More noticeable in females that become gravid with eggs, females carrying eggs puts additional pressure on the swim bladder.

It is more of a condition, rather than a 'disease'.
