Default losing fish

hi susanagnes and all
ok here goes the details.
I have 150 litre tank with 25 kgs of live rock in it, 4x T5 tubes (2x marine and 2x lifeglo), 2x heaters (in case of breakdown on 1)
2x fluval 205 filters with the filter medium and foam scrubbers, no carbon because we had an initial disaster (lost all the fish 7 of them) and were informed that the carbon had dumped all its waste back into the tank because we hadn't removed it after 2 weeks. Told to put it back in when tank water started to get a yellowish tint ( ?) it hasn't yet !!
We also have a UV on the return side of one of the fluvals.
There are no external signs and the first fish died about 4 days after we bought him. He was an algae eating Blenny, bloody misreable looking so we called him victor.
We have steadily lost fish from then on but they die real quick, literally fine in the morning then 4 hours later dead and half disposed of by the cleaning crew.
Just cant see the problem and starting to get abit down heartened

hello mate, right, ya set up seems ok to me so now we need to know temp/sg/water params ie no2 no3 phos etc/ have you got enough water turnover/flow[salt water doesnt hold o2 as much as fresh]/list of livestock/could there be any hidden critters such as crabs,eunice worms, lobster,pistol shrimp etc/ do you use ro? if so do you make it yaself,buy it from lfs/ do you check ro before adding it to tank the Qs are many but itll help us to help you

how many fish have you had in there at the same time ? to have 11 die on you in three to four weeks , there seems to have been a lot of fish in there ,for a 150 litre tank holds about 30 gallons water once you put the rock in
