are heaters in a marine tank ok to be exposed or is it better to have some sort of guard round them?

yes , espically if you have lots of liverock piled up, heater gaurds only about 3-4 quid

Is a heater better laid on the sand floor or better to be attatched to the back of the tank like a tropical tank? Bearing in mind that the tank has a bumpy lumpy backing on it!

Yes definitely use a guard.
I'd put it on the end of the tank, not on the sand as you can get a build up of heat and that will mean a false reading, resulting in the heater not working as it should. If you have a sump filter then that would be the ideal place to put it or Hidor do an inline heater, if you have an external filter.
